SIA Application Form

The madressah encourages on-site extra-mural activities as well as excursions to places of interest as part of the learning environment. All reasonable precaution will be taken to ensure the safety your learner. The madressah does not, however, accept the medical cost, however sustained. In the event of an emergency, please provide primary person's contact details:

I, the undersigned, being the parent/guardian, of the learner whose details appear on this page:
1. Do hereby verify that the particulars furnished are true and correct,
2. Accept that this application does not guarantee my learner a place at the madressah,
3. Undertake that, should my learner be accepted, that I will abide by the Code of Conduct and rules, as may be amended from time-to-time, of the madressah at all times,
4. Undertake that my learner will respect his/her teachers, treat them with dignity, and carry out all reasonable instructions,
5. Undertake that my learner will take diligent care of all textbooks given to him/her and treat it with due care and respect,
6. Sign the debit order form and commit to enure that all madressah and textbook fees are settled timeously, and
7. Do herey absolve the SIA, its council, Manco and members, Madressah prinicpal, teachers and staff, and their agents, from any claims, whatsoever, that may arise in connection wit the loss, damage to property, or injury to any learner, howsoever caused. I understand and accept that all activities, tours, excursions, extra-mural activities, shall be undertaken at the learner's risk an hereby designate the Principal an/or any person nominated by him or a person acting on behalf of SIA to act in loco parentis, on my behalf, in the full knowledge that they will nevertheless take all reasonable precautions for the safety an welfare of my learner. I will remain responsible for the payment of all medical costs, where applicable.
